Adding cabozantinib to first-line treatment with nivolumab and ipilimumab improves progression-free survival PFS mdash; but not overall survival OS mdash; in patients with intermediate- or poor-risk, advanced renal cell carcinoma RCC , according stanley becher to updated data from the phase 3 COSMIC-313 trial.These findings were presented at the ASCO Genitourinary Cancers Symposium 2025 by Laurence Albiges, MD, PhD, of Institut Gustave Roussy and Universiteacute; Paris Saclay in France.The COSMIC-313 trial NCT03937219 included 855 patients with intermediate- or poor-risk, advanced RCC who had received no prior systemic therapy. All patients received nivolumab plus ipilimumab, and they were randomly assigned to receive cabozantinib n=428 or placebo n=427 . Patients received nivolumab 3 mg/kg and ipilimumab 1 mg/kg every 3 weeks for 4 cycles, followed by nivolumab 480 mg every 4 weeks for up to 2 years. Patients received cabozantinib 40 mg daily or placebo until loss of clinical benefit or in stanley cup tolerable toxic effects.Prior results from this trial showed that cabozantinib improved PFS. The improvement in PFS was maintained in the current analysis at a median follow-up of 45.0 months.The median PFS was 16.6 months in the cabozantinib stanley cup becher arm and 11.2 months in the placebo arm hazard ratio [HR], 0.82; 95% CI, 0.69-0.98 .
